Story 1: strange calcitonin

One day in the late 1990s, Qi Lin received an elderly woman with diarrhea in the outpatient department. She had a lump in front of her neck. Before, she went to the digestive department because of frequent diarrhea: the thyroid function was normal and hyperthyroidism was ruled out; Enteroscopy found intestinal polyps. After electric resection, the diarrhea still did not improve.

After a detailed understanding of the incidence and treatment process, Qi Lin found that the old woman's cheeks were red. Combined with the symptoms of diarrhea, he thought of the "calcitonin" secreted by medullary thyroid cancer, which may cause blush and diarrhea.

Calcitonin is a very remote medical knowledge. It was only introduced in university textbooks, and there were almost no actual cases. The hospital did not have the conditions to detect calcitonin at that time.

Therefore, Qilin mobilized patients to the Union Medical College Hospital to test calcitonin. The diagnosis was as like as two peas.

Story 2: who kidnapped blood pressure

Once again, Qilin received an overweight middle-aged male patient in the outpatient clinic. He had been suffering from hypertension for several years. He was recommended to see Dr. Qilin.

After understanding the medical history, it was surprising that the patient was taking as many as five antihypertensive drugs, and two or three were the maximum dose, and the blood pressure was still poorly controlled.

After consulting the previous examination records, it was found that the blood potassium was low, so it was suspected to be hospitalized with primary aldosteronism.

After carefully studying the patient's condition and indicators, Qi Lin guided the ward doctors to check and get the diagnosis. Finally, he only took two kinds of antihypertensive drugs, and the general dose, the blood pressure was perfectly controlled, and the blood potassium gradually returned to normal.

Story 3, type 1 diabetes?

During a ward round, Qi Lin met a twenty year old female patient who had diabetes for seven or eight years.

The patient was hospitalized due to poor blood glucose control. She was diagnosed as type 1 diabetes and received four injections of insulin at ordinary times. However, the girl was thin and did not pay attention to her diet and exercise, "as if" she had type 1 diabetes. 

Qilin carefully inquired about the family history of the patient and the onset of the disease. He instructed the chief physician to do the relevant examination, excluding the diagnosis of the previous type 1 diabetes,changing to one injection of insulin before bedtime, taking the oral hypoglycemic drugs during the day, simplifying the treatment regimen, improving the compliance,and guiding the diet and exercise to control the blood sugar. The blood sugar has been controlled well during the follow-ups.

Story 4: lost and recovered patient

This is a female patient in her 60s. She recently came to the hospital because of high blood sugar. As soon as Qi Lin saw her, he feltlike getting a treasure.

It turned out that the patient worked as an elevator worker in the hospital ward building more than 20 years ago. At that time, when Qi Lin first saw her in the elevator, he felt that her body was unusual:

Her chin has thick edges and corners, the bridge of her nose is wide, and her finger joints are thick.

Qilin suspected that she might suffer from "acromegaly" caused by pituitary growth hormone tumor, but he was embarrassed to mention it to her for the moment. When Qi Lin summoned up the courage to talk to her, she had resigned from the hospital and disappeared.

In a flash, more than 20 years had passed, and they unexpectedly met again. The feeling of joy arised spontaneously. Qi Lin's mood was like the recovery of his long lost baby.

Qilin talked to her about his judgment in that year, and the results were verified one by one. Although so many years have passed, her family and herself are not aware of the obvious deformation of her appearance, hands and feet. The patient was very willing to cooperate and went to Yanhua hospital for in-patient examination.

After hospitalization, after imaging and hormone examination and determination, Qi Lin's judgment many years ago was verified. Due to the special location of intracranial lesions, the operation was difficult. Later, it was successfully resected in the brain surgery of Union Medical College Hospital. Story 5, diabetes - type 1? Type 2?

In the late 90s of last century, Qi Lin learned from the data that an Israeli doctor relieved 11 cases of newly diagnosed type 2 diabetes with insulin pump, so he began to synchronize with the top domestic experts in China to carry out the new research project of insulin pump therapy for type 2 diabetes, and finally passed the famous expert evaluation of endocrinology in China, received high praise.

Although limited by hospital conditions and research equipment level, the idea of this research topic is completely consistent with Israeli doctors and domestic top experts, and the results are also very similar. Finally, it has become the most important research achievement in the field of diabetes in China in the past twenty years. It is written in the guide to diabetes prevention and treatment in China. It also confirms the bold speculation of Qi Lin and experts in their research.

Story 6: lecture hall and night school

In the long term contact with diabetic patients, Qi Lin found that many people had a very poor knowledge of diabetes, the disease was not niticed until complications, resulting in physical and mental distress and financial pressure

Qi Lin urged the hospital department of endocrinology to establish a lecture hall on diabetes health education, and regularly organized propaganda and education work to guide residents to measure blood sugar, insulin and prevent the occurrence of diabetic complications.

Sometimes, Qi Lin will personally go to the community and give lectures to the people of miles around.

Qi Lin said: "I am willing to give you a lecture. I dare not say how much influence it has in the academic field, but I dare say that we have done it when we say that the service level has the greatest impact in the region. To train doctors and educate patients, what we do is unmatched by the third class hospitals. "

With the emergence of new media, Qi Lin saw the advantages of online communication. He registered the endocrine official account and set up a group of patients WeChat, awfully busy.

From the ;lecture hall to the night school of diabetes, and then to the official account of WeChat group and Department of Endocrinology, under the efforts of Qi Lin and others, under the support of information department, Yanhua hospital has already diversified in the chronic disease propaganda and education.

However, Qi Lin's steps did not stop. Next, he aimed at "Internet +". Take advantage of "Internet +. to do a good job in out of hospital management of patients. Therefore, the technical level is not the most important in chronic disease management. What is important is to improve the literacy of doctors and patients. Qi Lin was born in May 1964. After graduating from Capital Medical University, he was assigned to the Beijing Yanhua hospital in 1987. He studied at the Department of endocrinology of Concord Hospital of Endocrinology Department in China. He studied at the Joslin diabetes center of Harvard University, Beijing University of medicine and 301 Hospital.He served as the director of the endocrinology department, assistant to the dean, and is now the deputy dean.

Qi Lin was selected as an advanced individual of Community Health Science in Beijing in 2009. In 2011, he was awarded the top ten endocrinologist of the national endocrine and metabolism branch of Chinese Medical Doctor Association, as well as the advanced worker of Beijing health system and the top ten diabetes teacher of Beijing Diabetes Prevention and Treatment Association

. He was also elected to be the Standing Committee or member of several professional associations of the national level,and Vice Chairman, Standing Committee and committee member of several municipal professional associations.
